A Clinical Study on the Surgical Complication of Clonorchis Sinensis Infestation in the Hepatobiliary System

Abstract
Surgical complication of clonorchis sinensis was occasionally developed and operative mana-gement for it performed in Busan and Kyungnam. But operative findings, clinical manifestation and laboratory data of the disease were rarely discussed in Korean literatures.
According to above mentioned viewpoints, 45 cases of clonorchis sinensis complication among 587 hepatobilary diseases were studied. The results are as followings.
1. Liver fluke induced surgical complication occupied 7.6% of all the surgical disease of hepatobiliary system at the department of Surgery in Busan Gospel Hospital during 3 years and 1 month (July 1972-Aug. 1975).
2. According to operative and hi stopathologicai findings, surgical complication of liver fluke could be classified as followings.
1) Cholecystitis
2) Cholecystitis and cholangitis
3), Cholangiohepatitis (Hong Kong disease)
4) Obstructive jaundice and abscess formation
5) Primary hepatoma
3. It was a very interesting view that cases of high output bile through T-tube (1, 500cc pr more a day) had remarkable adenomatous hyperplasia of intrahepatic bile duct and periductal fibrosis on the histopathologic findings.
4.On the histopathologic findings, it could be considered a carcinogenic factor of chola-ngiocellular, carcinoma because some cases revealed atypicality and ademonatous hyperplasia of-ductal epithelium and hyperchomatic changes in the nucleus.
5. Mutiple irregular filling defect on liver scanning corresponds with the location of winter-tree-pattern on T-tube cholangiography.
